I recall the guys mentioning something about "It's impossible" and "Going postal" existing in some alternative form, possibly intended for vanilla release. Really different from the current versions however.
And then Derelict is already in LD as well. That makes it 3 maps.
Possibly some other maps as well. Expander too existed before vanilla was out.
Overall, it seems that PP has loads of scrapped content that just got polished
When it comes to quick kick, it behaves very differently in .99 and I think even up to 1.1 where it just does a really slow kick with the "mighty foot engaged!" text while lowering the weapon automatically for the kick itself.
This just means that Q-kick evolved a lot during that time and 1.3d behaviour is indeed a leftover bug from all the last minute changes it had.
In future versions, there are also subtle speed differences between Q and regular kick, not to mention that it already feels off to have it in two different places, and finally: why does the other one need a hud text for the same action ?
Not to mention that it all could have been some remaining balancing from the times when enemies took much less damage to die.
