On first pull of a shotgun's trigger on a given map, for a frame or two the muzzle flash appears in the upper left corner of the screen.

At least I think that's what causes it. It's not easy to tell since it's barely a flash on the screen.

I'd provide a screenshot but it's incredibly difficult to time the screen press. At least it is for me.

I have not noticed that but I wonder if it is related to the recent weapon smoothing interpolation code that was added.

Almost certainly. The part I can't figure out is why the muzzle flash is pre-loaded and has to "move" into location, but only exactly once and then never again.
