A while ago I tried to play Nam via EDuke32, and noticed that the M16 (starting weapon) has a way worse accuracy than if playing Nam directly. In vanilla Nam, the M16 has pin-point accuracy even over large distances and the auto-aming makes killing enemies very easy. In EDuke32, it seems like the weapon accuracy is the same as the Duke Nukem 3D pistol, in other words, horrible, with bullets spreading all over the place. It makes an already hard game virtually impossible, which is a shame because I really like Nam and I was hoping being able to play it with EDuke's advanced features. I read on the Steam forums for Nam that other people have a similar experience when playing on EDuke. Any ideas?
Edit: The HUD icon for health is also replaced by the icon for rockets, for some reason.

Trying playing NAM with the oldmp branch. A friend and I played through it without any of the issues you've described using that version. Thought it's a bit outdated, it seems to support it best in my opinion.

Thanks for the reports. Are there any other differences between NAM and EDuke32's support for it?

I tried as you suggested but there's no change, sadly. Oh well, I'll just play it normally.

Which version of EDuke32 are you running? As far as I remember, there was a version few months ago where the autoaim was broken.
Anyway I know that the pistol/M16 accuracy is affected by looking up and down. If you look in center and aim for an enemy, no matter how far away or how high/low he is placed, the weapon has deadly accuracy and will always hit him. But if you are looking up or down and shooting, the accuracy is much lower.
Also are you using 8-bit software render or 32-bit polymost/polymer render? If I'm not mistaken, the render chosen also affects the aiming, for example in software the pistol/M16 has better accuracy.
As for HUD health icon showing as rocket icon, it is because in Platoon TC, NAM and WWII GI, the small medkit (which EDuke32 uses as icon for health in the HUD) is replaced by a rocket. This is also why you won't find any small medkits in the game, only large and portable medkits.

I have tried both the newest version with and without Polymer as well as the old version that was suggested.

The behavior of the M16 (and pistol in Duke) is such that if you're aiming exactly at an enemy, the shot will hit them, but if you aren't, it will have a bunch of spread. It should be ridiculously accurate if you use it correctly.

The thing is, I played Nam multiple times since it first came out, and the M16 always had perfect auto aiming no matter the distance. Only once I played it via EDuke did I notice how much less effective the weapon has become all of a sudden.
I just tested playing Nam via Dosbox and the M16 is as it should be, with perfect accuracy. Every enemy will be dropped in two shots as long as the crosshair is within half an inch of him on the screen.
