The only thing wrong with The Hive was Duke's reaction to the Holsom Twins' predicament which was a bad joke that didn't land because Duke would never say that. And the wall boobs I guess which were just stupid. The former is Gearbox's fault because 3DR had no dialogue written when they shut down. The latter....I don't know where that came from or why. It's just bizarre lol.

nah there's plenty of wrong beyond that.
Duke's reactions to mercy killing women are pretty weird, some are in line with what he would say in duke 3d, and others are just him being a piece of shit ("why do they always take the hot ones", right after mercy killing another woman and apologizing for having to do that, a genuinely humanizing moment gets ruined with a bizarre reaction completely at random)
The women themselves also can switch at complete random from being horrified at their predicament to cracking jokes ("I thought it was safe to swallow" and "too many yager bombs" most notoriously)
Finally, the anus doors and Duke's reaction to them (worth pointing out because Prey 06 did this shit as well and did it way better, but then again Prey 06 was eons better written than DNF)
As a little bonus I could say Duke's reaction to the webbed soldier at the start of part 2 felt rather mean spirited ("thanks for hanging out"), I feel Duke should have either been silent or angry by that point.
All these issues basically culminate in that the Hive feels like a tonal clusterfuck. I have no clue if the game's trying to be funny, serious or something, whatever. It doesn't help several moments just make Duke seem like a dick yet other times an attempt to make him likeable is made.
Hive is essentially a microcosm of all the tonal and writing problems within DNF in hindsight, and it's strange because so much of it is very very easy to fix, but then again 90% of the writing problems in DNF are easy to fix because most usually end being aggressively unfunny jokes that sometimes halt the action (FUCK YOU DYLAN) and simply needed to be cut. Seriously, 90% of the humor in DNF is Seltzerberg tier and that's putting it kindly.

Also, fuck you to whoever made bugballs cause interference when using duke vision. That's fucking terrible, TERRIBLE design when you consider that the balls don't illuminate much and that the level's rather dark, so you need duke vision on to see stuff.

These are all writing problems which, again, are Gearbox's fault. I didn't remember any of them except for "It's better this way" so I didn't mention them, but I agree the writing is atrocious. But aside from the writing and dialogue, there's nothing wrong with The Hive. It's no different than Duke3D. I don't think the Hive itself is bad conceptually, it's the execution i.e. all these jokes that clearly didn't exist prior to the triptych version and don't fit with the serious dark tone it seems to be going for that are the problem.
I should note GBX didn't write the game, Triptych did. The only change that I believe GBX did for this section (based off a random comment randy made on the now infamous july 2017 livestream, that hive "used to be worse") is changing Duke's original reaction for the twins' death. There's an unused duke line in the game that (based off the naming) fits right in at the end of the conversation which is him nonchalantly saying "What a waste", zero emotion, nothing. Complete asshole and bordering on sociopathy. I strongly suspect that is a GBX change since the actual used line for the twins death doesn't match the naming of the rest the hive duke line files (it's called duke_twinsdie01 not duke map07c like the rest) but also the audio has weird artifacts only present in one other duke line - the borderlands joke, a joke which I also strongly suspect replaced an earlier joke (bioshock big daddy, also has an unused duke line and unused static mesh that has no polys from behind curiously) The "what a waste" line is crystal clear, and matches the audio quality of the "you're fucked" line, possibly implying that gbx made a last minute change to duke's reaction to not make him such an asshole. Shame they didn't change all the other stuff.

There have been hundreds of peoples having worked of the game and the Hive by the way, and it have been a pass for all of them, but anyway Gearbox and Triptych are mostly composed of former 3D Realms employees so there is some sort of continuation of Duke's legacy in both Borderlands gamesand Duke Nukem games since DNF

For the Psycho head having replace Big Daddy head, yes indeed, it was revealed by the guy having made the quality control who made the replacement, and he was ordered to do so by 2K
