I play duke 3d on let's rock and am comfortable with that. I am doing my first playthrough of Blood now, playing on pink on the inside. Yet it is easier than I expected. I know that the diffculty bug is now fixed, but was it how it was back then?

I would say "pink on the inside" is easier. I usually play on "lightly broiled" or "well done". I would say lightly broiled is about the same as "let's rock" in Duke. It's the default skill level selected so I kind of see it as "normal".
