If you are talking about the levels in the game and not custom made...

I think E2L5 (OCCUPIED TERRITORY) is quite hard, since it has many battlelords and commanders, specially in higher difficulties... And not that much ammo to compensate.

Official maps: The Queen (E4L10) - Lots of enemies in tight spaces (including two Battlelord fights with little space to wiggle around), and there's also the Queen battle which not only is underwater, but you also have to deal with Octabrains and Drones on your ass; all while trying to avoid her attacks and also getting drowned.

User maps: Zarathustra, the second-to-last level from Blast Radius. You start with no weapons, lots of enemies around you wanting a piece, purple lava everywhere, very precise platforming sections and also trying to figure out where to go next. It's such a ballbusting level that it became a VERY divisive level - you either love it or hate it. I personally thought it was a good level, even if my ass was sore from getting kicked so many times; but I can understand why people would despise it.

As for standalone levels, the hardest I can think of is Duke's Nightmare - which was designed for coop, but it technically can be played solo. It's just really difficult, given it's slaughter map nature.

I'd partly agree with Dr. Panico regarding The Queen being the most difficult from original maps; also, I think Warp Factor deserves a mention as it can be really tricky pistol starting since you're pitted against some drones right at the start. For first time players, The Abyss also can be difficult due to easily getting lost there.

Zarathustra wasn't as hard as the final level of Blast Radius IMO, I also liked it a lot more than the latter. It just can be very inconvenient at the beginning, but settles relatively quickly (given its whole length). As for other user maps, then probably something like Glitches in the System which I gave up on even despise following a video tutorial :D

Beside the maps mentioned, some Billy Boy maps such as Lighthouse are probably among the hardest ones (and also longest).

Anything made by Zaxtor.

The firsy level of the exclusive episode of the PSX version (if we are talking about official levels) is pretty hard

I definitely agree but I really didn't find those maps enjoyable-difficult, rather they were (to me) annoying-difficult, if that makes sense.

Now on the other hand, the Billy Boy stuff already mentioned was way enjoyable, even in the really difficult sections. I got a sense of accomplishment from figuring out puzzles, finding really hard secrets, winning battles and so forth. There's a balance, and imo mappers like BB and ck3d and a bunch of others have that nailed :)

When I play a map, with a great one I feel like I'm playing against the creativity, intelligence and sense of fun and humor of the mapper. All very much just imo, of course.

Yeah. Zaxtor seemed to be aiming at a type of unforgiving difficulty typical of retro gaming, but I think he went a bir too far in some maps; to the point even he seems to struggle at his own levels sometimes (just check his Trequonia playthrough). Heck, only a handful of people managed to beat Oblivion without cheating at all!
Billy Boy on the other hand managed to strike that sweet balance between difficulty and fun. It feels so good when you beat one of his maps, and you might actually replay his maps later on.
