Download the following zip, it contains a simple test map and a per-map art file.

Launch the map, save and load. After loading, the textures rotated by 90 degrees (i.e. the ones with cstat 4096) will become HOMs. This does not happen once the per-map tile art file is removed.
Furthermore it does not require the tiles to actually be used for the bug to occur.
Bisection indicates that this was always broken ever since the feature was introduced.

The diff in this archive fixes the problem:

The rottile array wasn't being backed up and reloaded with the other arrays in the mapart functions.
